Public Offer

1. General Provisions

This agreement is a public offer contains all essential terms for the provision of services.

2. Subject of the Agreement

The Provider undertakes to provide the Client with the following services:

• Consultations on overcoming addiction
• Development of individual rehabilitation programs
• Consultations for relatives of persons with addiction

This document is not a contract for the provision of medical services and does not replace professional medical care.

3. Price and Payment

Service prices are determined according to the current price list. Payment must be made before services are provided.

4. Confidentiality

All information about the Client is confidential and will not be disclosed to third parties without consent.
